(iv) DRIVING LICENCE
(v) UNIQUE IDENTIFICATION CARD

(2) Any ONE of the following documents for Residence proof---

(i) PASSPORT
(ii) ELECTION CARD ISSUED BY THE ELECTION COMMISSION OF INDIA
(iii) DRIVING LICENCE
(iv) UNIQUE IDENTIFICATION CARD
(v) LAST PAID ELECTRICITY BILL(IN THE NAME OF THE APPICANT OR HIS PARENTS OR SPOUSE)
(vi) LAST PAID TELEPHONE BILL( IN THE NAME OF APPLICANT OR HIS PARENTS OR SPOUSE)

(3) Any ONE of the following documents for proof of place of business –

(i) PROPERTY CARD OR PROPERTY TAX BILL OF LAST YEAR
(ii) COPY OF INDEX-2 ISSUED BY THE SUB-REGISTRAR OF STAMP DUTIES.
(iii) AGREEMENT OR LEASE DEED DULY EXECUTED IN CASE OF THE RENTED PREMISES
(iv) CERTIFICATE ISSUED BY THE LOCAL AUTHORITY IN RESPECT OF SHOPS AND ESTABLISHMENT.


The above documents required—
In the case of Proprietor--- Proprietor’s himself or his parents or spouse.
In the case of Partner--- Any one of the partner
In the case of Company Registered under Companies Act—Any one of the Directors
In case of HUF--- Any one of the members of HUF

(4) Following documents ---

(i) Certificate issued by the Registrar of Companies and Articles of Association in case of Company.
(ii) Partnership deed in case of Partnership Firm.
(iii) Instrument of HUF in case of HUF.
(iv) Copies of Challans for the payment towards amount of security i.e Rs. 10000/- in case of State Vat Act and Rs.10000/- under CST Act.
(v) Rs. 25000/- as advance vat tax, if Sales or Purchase turnover below 5 Lakh.
(v) 2 Passport size photographs with original documents as stated above.
